OWP application question

I'm going to be sending our inland spousal sponsorship application package together with the OWP.

My question is do I need to attach only the OWP application form (IMM 5710) together with my spousal sponsorship package? Or do I need to attach the whole OWP package with the spousal sponsorship package?

The package includes :
Document Checklist [IMM 5556] (PDF, 206.28 KB) February 2015
Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker [IMM 5710]
October 2015
Statutory Declaration of Common-Law Union [IMM 5409] April 2015
Use of a Representative [IMM 5476] June 2015
Instruction Guide [IMM 5553]

You don't need to include the instruction guide, Stat Declaration, or Use of a representative form .

Actually, you only actually need IMM 5710, because you are eligible based solely on the Inland application.
Be sure to pay the full amount for the OWP ($255 total).

Those that are applying for a regular OWP, probably need the other items in the package.

Hi! I know the post is a bit old, but I have a concern about the form. What did you write on the section of Details of intended work in Canada from Q2 - Q4? "Not Applicable"? Cause I cannot leave those empty.

So just to confirm - if we're sending in an OWP with PR application, the only OWP form we need to complete is 5710?

And with documents, do we need to include a duplicate copy of passport etc or can they use the documents we've included in the PR application?
