What did you write for the "company" section of the address??

You can probably write your employer's name and give your residence/mailing address. Not sure why they ask for it in the first place.

I'd really like to have a copy on hand

I printed a copy using my laser printer and printed on both sides of paper, that way the whole book was printed in around 34 pages.
