ordering GCMS notes

Can someone help me order these im stuck at Section B

which checkmarks do i check, Im sponsering my wife in Jordan and her file is in the AMMAN VISA office.

Check the Electronic file and status of application.
It is written in the form that if you apply for Physical file (Paper file) it will be delayed your processing. If you apply for electronic file, it will not. So check the Electronic file and status of application.
I did the same thing and received the notes within one month.

Thanks one last question when i fill out the form 5475 " AUTHORITY TO RELEASE PERSONAL INFORMATION TO A DESIGNATED INDIVIDUAL "

do i have to sign being im the spouse ?
