The Option C printout is a report from Revenue Canada on your sources of income in the past tax year.

If you cannot get one, having not worked/filed taxes in Canada for so many years, you can present your tax return from the US along with your payslips for the last 12 months. Include a letter explaining why you are not including the Option C. Others have done this successfully.

Thank you Charlie, I am living here in US since 2006. I will include my US tax returns for this year and will also explain why I am not including the Option c'. I do not see any reason to include Canada tax returns since I havent lived there in 5 years. :)

If you HAVE filed Canadian tax returns, you really should include the Option C. If you haven't filed any because you've been out of the country, then yes, use the alternate means suggested.

Thanks for your help, like I said I have not filed a Canadian tax return in 5 years. :)
