Option C printout

Do I have to include the cover page? It doesn't have any of my tax info, just my name and some basic info..

I may have thrown it out, not thinking that I needed it

Hi, you need to get an option C print from CRA. If its not provided with the application it may return back to you, they will label it as incomplete.

I did get the option C printout. I was wondering (it came with 3 pages), whether the first page, which is pretty much almost blank with none of my tax info (only name and my address etc.) is necessary?

3 pages?... ummm.. not sure about 3 but I only got 1 page.. but if you got the 3, might as well include them also.. just to be on the safe side

Has anyone else NOT included the first page (which is essentially blank) of their option C printout?

no need to include the cover page. just the NOA part and the T4 page. (they are in different format than what we get from CRA usually)

I didn't! It's just so that the window in the envelope could have the address. I burned it (We don't shred here, we have big bonfires! haha)
