This is a follow up to your request for a Work Permit Support Letter.
In order for us to issue a Work Permit Support Letter, please provide us with the following additional information and/or documentation:
Please confirm your current residential address (i.e. where you are physically living now).
Please confirm if you are currently employed. If yes, please provide your position title, the name of the company for which you are working and the full address of your current workplace. Also, please provide a copy of your two most recent pay stubs, if you are currently employed with this same company.
Your current contact information (current address, phone number/s and email address)
Please confirm your current activities in Ontario (If you are not currently employed.)
If your contact information has also changed after nomination, you will need to send us a Change of Personal Information form in order to have your address and/or contact phone number updated on our system. The Change of Personal Information form is available on our website at:
under “Forms and Guides”, under the “Other Important Downloads”. Please complete this form and send to us so we can officially update your application details.
Always remember to quote your file number in any correspondence to our office.
