Open work permit

Hello Folks,
I want to submit an application for Open Work Permit online. My spouse is in Canada as an International student. After creating myCIC account, I responded to the questionnaire to determine my eligibility, then, I received the required documents that I need to upload. But, I am surprise that Family information form (IMM 5645) is not part of the forms in the uploaded links created in my account. So, I want to know if family information form is required for open work permit or not.

Same thing happened to my wife they didnt require family form.. i ddnt upload the form and i got accepted. So i guess its fine

Thank you for your response, did you apply while you were in Canada, and she was still in your country?. and how did you show means of financial support?

And please bro, I want to ask if you submit her application for open work permit at the same time when you submit your study permit application. Or you submit her application separately when you have already got to Canada.
