open work permit with inland pr spouse app

I am sure the answer is somewhere but I have searched high and low.
Can someone please tell me where to find the OWP application forms that I would submit with the spousal PR application..
I am from Australia in NS and my wife is Canadian

one more thing.what would i check at question 3?...do I just need to send the 5710 form extra to my pr app and thats it?

There is a checklist of things you need to attach to the application and it is $150.00 for the work permit. You can pay this online then proof of payment needs to be enclosed with this application.

Question 3 can only be work permit with new employer unless you have a work permit at this time. When you get down the page a little, it will ask you what type of permit you are applying for which is Open Work Permit. Good luck with everything.
