Open Work Permit IMM5710 - Common Law Sponsorship

Hello everyone!
I am doing my open work permit together with my common law application. I would like to know in Question 3 of the IMM5710, which option do I select?:
- A work permit with the same employer;
-Restoration of your status as a worker;
-An initial work permit or a work permit with a new employer; or
-Temporary Resident Permit ?

Also, I would only have to attached the form (IMM5710) and the fees, but no the whole package for Work Permit, right?

Initial work permit. For employer, show `unknown' (or something similar).

Make sure you pay the full amount for the OWP, which is $255 ($155 + $100)

Send everything together in the same package, with the OWP clearly labelled as an OWP for an INLAND (SCPLC) applicant.
Once CIC receives the package, they will route the OWP application internally.

Sorry, I have a question. What should I put on the section of Details of intended work in Canada from Q2 - Q4? "Not Applicable"? Cause I cannot leave those empty.

how about Q5 : Duration of expected employment? Do I leave it blank?
