open work permit application with permanent residence file number

I already have a permanent residence file number from CIC and just finished my medical exam 3 months ago. My work permit with my employer is valid until next year. I just want to know if i can apply for OPEN WORK PERMIT? and where can i find the application form?

to expire within 4 months.
