Ontario returned my package for being incomplete, as I somehow forgot to print out and add the last 2 pages of the Application Form to the application package. Those 2 pages include the declarations, authorizations and more importantly signatures and dates of the primary applicant and the dependent. The funny thing is I never included that page first time around so I never added a date to my application form before. Now when I add the date as 12 Oct 2015, would that create problems as my application was first received by Ontario on July 20. So my bank records are 6 months prior to July 20. Then again, the application form would be dated October 12. Should I date it July 16, the date I sent the application package for the first time, or October 12, the date I am re-sending after my package was returned? I just don't want to encounter a rejection based on "Your bank statements do not include the last 6 months before application date". Thanks.
