But there is 2 check boxes only in this form , one for principal applicant and one for spouse or child 18 years or older , so in this cases which box to check for a 3 years old child , I think this form is not applicable for children under the age of 18 , moreover there is no signature on this form to be signen on the child behalf , appreciate your reply

This form needs to be filled by Principal Applicant and Spouse and Children over the age of 18.

2. The form that I am talking about is form Application for Temporary Resident Visa (IMM 5257). This one needs to be filled for all applicants even minors. This is the form that needs to be signed by Parent in case of a minor

This is the only form that needs to be filled for a minor.

and dear VFS ....stands for

From India, and VFS (Visa Facilitation Service). I can submit my OWP application at local VFS center. They will collect all the required docs and send them to canada high commission, New Delhi,India.

Yea, TRV is must. When your OWP approved, they know that you need TRV as you are applying OWP from outside Canada. So, they will issue TRV i.e. Physically stamp on your passport.

Whereas for candidates in Canada, they don't require TRV. Just OWP application is sufficient.

For temporary visitor visa you need to fill the form which is included in wp application. The link you referred is for those who only want to apply for visitor visa not wp, don't be confused and just follow the instruction in the wp application guide.

I am pulling my hair out for this one and I desperately need your help and advice!

I am preparing my application for Open WP (Ontario) through Abu Dhabi. My family will accompany me and the embassy says (http://www.cic.gc.ca/english/pdf/kits/forms/IMM5645E.PDF) that I need to apply for "Open Study Permit"s for my kids aged 6-18. Has anyone done this before and which form do I need to fill for the kids?

Primary and secondary school (age 6-18): open study permit. The application can be made using the same form as the accompanying parent's study or work permit. You do not require an acceptance letter from the child's school in Canada.

Which form are they referring to? If its the "IMM1295E-Application For Work Permit Made Outside of Canada" how can I use a form meant for a work permit application as a study permit application for my children? I am stuck with just this small issue. Please help.

as far as funds are concerned ..Ontario PNP sent a letter first time in Feb for five categories ..you remember?... in that letter funds are mentioned for all family member size..like for 1 for 2 for 3 for 4 for 5 ....you must refer that letter....

i made a mistake in my 3 years child information unconsciously about his appearance like his hight color of eyes or spot on body...

1- indicate how long you plan to stay from .... to ..... and all are mandatory fields
2- name and address and relationship of the person iam going to visi and also are mandotory fields

so in this case I agree with Mryam that we should stick to the checklist of th WP ,as this application is not valid for our situation

You need to fill the form for the TRV the main barcode form for your family members, Its the first form in the link I provided, I am saying this because I have filled these forms when applying for visa in the past for dependents along with my WP. Nonetheless, you can confirm with VFS about this, incase they have changed the rules. But as far as I know its needed for TRV for your family members. Your family will be issued a Visitor Visa not a Work Visa

Hi Hadeel,

Do you have a VFS office close by. If you do then I would suggest you either call them and ask them for clarifications regarding spouse and children visas and if you need to fill that form TRV (5257) for them. Also please check the website relating to your visa office, from what I read it says you have to include separate applications for each of your family members along with your applications.

1. If you have to fill in the form and the TO and From dates would be the two year period that you are applying for, say 31/12/2012 - 29/12/2014 - it accepts the date I checked.

2. The address - I put in NA and it accepted it.

3. About form IMM5257 - Schedule 1 - I dont think you need to fill it, If you check the form IMM-1295,
it says at the bottom after Question 6 If you checked “Yes” to 3-6 you may be required to complete IMM 5257 – Schedule 1.

Also my advice to you is, first fill the WP forms for you and spouse - add your children's details in the family information form and wherever applicable. Then call the VFS office and clarify the above. I will see if I can find more information on the same. Give me till monday.

Although the cover page mentions that they support work permit application for me and my immediate family but the formal support letter mentions only my name and does not say anything about support of my spouse.

No, you only need to apply for your open work permit. This open work permit will allow your spouse to work as well. She does not need to apply for a separate Temporary Resident Visa.

If you were successfully nominated under the FSW Pilot, then you only need to apply for one open work permit. You will have to contact CIC to see if you can possibly get the fee back for the work permit application for your spouse.
