Not Valid For Travel means you cannot use this paper to enter and exit Canada..You need travel document i.e passport along with this to enter and exit..I have no idea about Surverillance code and once you get your PR you dont have to worry about this codes..good luck

it's something related to the medicals as far as I remember, and if I remember correctly it means that no medical surveillance is needed.
If there was an issue with it, you probably would have know about it by now.
