Hey Guys, Im getting pretty close to finishing my application but I'm not sure what to do with these forms

applicant forms
additional dependants / declaration form IMM 0008DEP
declaration from non-accompanying parent/guardian for minors immigrating to canada form IMM 5604
use of a representative form IMM 5476

sponsor forms
use of representative form IMM5476

Its just me and my wife with no family members so I am 99.9% sure that these forms do not apply to us
can I leave these forms out completely or should I submit them blank or with an N/A on them?

You are sponsoring your spouse, correct? Are currently living with her? If not and you are in Canada, then you can fill in the representative form so that you would receive any correspondence as well as ask for updates on the application. If you decide to fill this form, then the mailing address in IMM008 should contain the same address that is provided in the representative form.

If you are living together with your spouse, then this form is not necessary.

IM008 is an applicant form with my wifes costa rican address, the representative form should have my canadian address right?
I'm glad you brought this to my attention though, I didnt realize that I can be the representative.

I'm still wondering whether or not I can leave these forms out of the application?

In the IMM008 form, the "Residence Address" should be that of your spouse where she is living. The Mailing address should be yours (Canadian address) IF you are filling the representative form.


Regarding your original question:
IMM008DEP, since you are sponsoring only your spouse (you do not have any children), then this form is not required.
IMM5406 (Additional family info) This form MUST BE FILLED by your spouse. Even though her family members are not coming to Canada.
IMM 5476 (Representative Form) Fill this only if you wish to be a representative of your spouse - otherwise this could be left out.

Yes we filled out 5406, that form wasnt in question it was 5604 that I was inquiring about.
Thanks for the IMM008 info, I wouldnt have guessed to put my mailing address in there.

Just wondering, filling out my address on her IMM008 form wouldnt cause them to mail the visa or important documents that she needs to me would it? I assume they would contact us first before those kinds of things would be mailed?

There is a mailing and residential address. If your mailing address is different from the residential address, simply click the box (yes or no). They'll send everything to the mailing address. For us, we used my wife's parent's address...but they live in the same city, so no worries.

As for the OP, I provided an appendix listing all the forms and items on the checklist that were not applicable. I read that you can either add an appendix or include the forms with NOT APPLICABLE written on them. I think it's easier (and better for the environment) to simply add an appendix.
