The error message that you got is "The recipient's mailbox is full and can't accept messages now. Attachment by email cannot exceed 25M, so just split the files into 2 emails.

UCI or Unique Client Identification number is a number that IRCC gives you when you submit an application. If you have/had any visa (like visitor visa), you should mention that UCI in EE profile. UCI number is not mandatory for Medical Examinaiton. The following would suffice.
1. The Invitation to Apply letter (ITA),
2. Original Passport and one copy of it.
3. Passport size photos
It is good to have the reports handy if you had any medical history. Call your clinic before going there if you have a specific question.

I did the same after facing the errors of too big / heavy an email. However, I immediately got a response from OINP saying to send the same in Multiple Parts but to maintain the "PDF" format only.
So, I sent the mails as "Part 1 of 3", "Part 2 of 3" and "Part 3 of 3"
Post that i received a confirmation email the very next day. The email stated... "Thank you for your response. The documents and information provided have been added to your file. Your application is proceeding for further assessment."
Therefore I'd recommend that you follow a similar procedure.
You should just reply to the ADR email without changing the subject line. That's it. From what I read in this forum, there's no reply email.
