I applied for OHIP records last April 2014 and received it after 5 weeks.. I also used the form asking for MY personal records, and I did not pay anything. Hope this helps.

I did the same thing.. All forms in one envelope, no proof or ID whatsoever. Express post. After 5 weeks, we received our records individually or in separate mails.

My son and myself together, my wife separately applied for OHIP records by Regular Mail at the end of January. I got mine about after 5-6 weeks, my wife almost 4 months after the application. No Idea why ::).
I don't know if you need them for a RQ or just as a precaution, but I recommend that you ask for them as soon as possible anyway. I didn't need them until know and my oath is in 2 days. I hope I wouldn't need them till then :).

Yeah, I need those records for an RQ. I just received my RQ yesterday, but it still took the document a week to reach me.
I will request them now and will hope that I get them on time. Otherwise I will include a letter.

When I got my RQ I didn't even know you could get records directly from OHIP. I just called my doctor's office and asked them to write a letter stating the dates on which I had been seen by the doctor. I did the same for my dentist, eye doctor, chiropractor and the local hospital where I had received treatment. I was able to pick up all the letters within a couple of days and those are what I sent in with my RQ.

elfriend your response confused and scared me. i just submitted my OHIP records. so u recommend i should get those miscelleneous each clinic records like dental eye chriopractor etc as those are just 1 - 2 visits only for the 4 year period what value it will add on my RQ. i dont understand why cic is giving so pain. like this i will get older soon with stress. i had recently got my physical done and in test i found lot of issue in my body balance just name it colestrol hypertension thyroid etc
