does this mean outland Buffalo applicants can apply since i am eligible to apply for my PR card? (i had no luck with the midwives all of them are booked)

I believe this refers to inland applications that have received AIP (i.e. first stage approval).
 
I assume the same- once you have AIP inland with that letter you can apply for health card.

No - unfortunately it doesn't apply to you.

Since you are applying outland and will be living in Ontario, you will qualify for OHIP 3 months after you land as a PR.
