To Apply for your health card you need 3 types of Identifications as per
http://www.forms.ssb.gov.on.ca/mbs/ssb/forms/ssbforms.nsf/GetFileAttach/014-9998E-82~5/$File/9998-82E.pdf
these are my experience for applying for the health:
1- from List 1 I used Record of Landing
2- from List 2 I went for driver license written exam (You can walk in and take the exam) and got a paper showing that I have passed
3- from List 3 I used my country's Passport.
Given that you are only going for a short trip you can all apply at once. If you think you may extend your trip I would wait and do your application when you get back. Please advise taking into consideration the following scenario:
My wife, two kids (age 6 and 8), and myself will be landing in mid November 2017. I will get them settled in Canada. After receiving my PR card I alone intend to travel to my home country for 5 to 10 days max to sort out my properties and bank accounts and return to Canada.
My question:
Should I apply for OHIP for all four of us or should my wife and two kids apply while I apply after returning to Canada after my short trip?
I'm a bit confused after reading the residency requirement before applying. Any clarification would be much appreciated.
Thank you so very much for the clarification. I don't believe the trip will be extended but might be shortened to 3 to 5 days. I will apply all at once then as per your advice.Given that you are only going for a short trip you can all apply at once.
