OCI- printout of duly filled online Application form.

I will be sending my oci application by post.
As I understand, all copies must be self attested, does that mean I need to self attest the printout of online application aa well ?

Also , for minor born in India, can the child's supporting documents be attested by one parent, or do both parents need to attest the copies of child's documents ?
