Parent has to sign for minors in front of the officers , or you can ask the Officers on the Oath Day to make sure.

I see that i dont need to send the form for minor who is less than 14 years old but most were talking about sending the form for the minors. Was that for above 13 year old minors?
Anyone had minor less than 14 added to your application and you didnt send the form and received the certificate for the minor without any problem?

I didn't send anything for my minor son. The clerk said so in the ceremony herself. It makes sense too. People who take the oath sign it. Minors are not asked to take the oath. So they aren't asked to sign the form.

Parents are not required to sign or send the oath forms for minor less than 14 years old. The intructions are very clear on the pdf file you will receive. My son was 7 when we sent our applications and 9 when we took the oath and we didn't send his oath form.
