Pre-Assessed..
Yes
If your appointment is on May 12th, then you should be able to upload that day, via Webform the document issued by the medical center, confirming that you did the medical. So, you will be well within the 30 days time frame.

There is one line in my remedical instructions :

" Once your medical examination has been completed the Panel Physician will submit medical results to IRCC for assessment. To obtain a copy of your Immigration Medical Examination please ask the panel physician at the time of your appointment. "

Does it means I need not to submit any webform after the medical exam? Panel Physician will itself do it

Yes, you need not to send it, hospital will do. But as a precaution, you can send the documents which you get from the hospital, if your hospital takes time to submit the result.
In my case, I had done my re-medical on 8th April and the hospital is yet to upload it as they will take 2-4 weeks to upload it.
Mine 30 days ends on 18th April and on the 19th of April, my medical status updated from we have requested medical to we haven't received your medical result. I have sent the documents received from the hospital via web form on 18th April.

As @ag2020 wrote, you should simply send via webform the paper that the medical center gave you to confirm that you came by to do the exam. It will have all the necessary references the IRCC needs to locate the actual results that will have been transferred directly by the medical center to IRCC.

May I know your medical status now? Is it still "we haven't received your medical result", or is it passed? I got remed request on April 23rd, and the clinic has requested IRCC to link my previous medical exam done on April 11th. IRCC confirmed that they linked that medical results to my new IME number, but the portal still said "we have requested medical". It keeps me worried about my medical results...

May I know your medical status on the portal after completing the remedical? Mine is still "We have requested..." although I completed it on April 11th, and the clinic uploaded results on April 18th already.
