Hi Asha,
To go for medical tomorrow you should've been nominated recently and should've got ITA recently! You don't have to take medical now for last year's nomination! So, tomorrow's appointment is perfect. It's advised by CIC to keep medical last to get more time for landing, however you can get it done anytime from ITA to 90 days. After about 50 days from ITA would be ideal as sometimes PCC can take a little longer even up to 50 days. If you've booked for medical tomorrow, you may have it, no problem.

There will be no difficult forms to. You'll sign a declaration form, meaning you allow them to do the medical and use your results for immigration purposes. So, if they have to show your results to a specialist outside CIC, let's say a consultant, your declaration will allow them to do so.
Apart for that, there'll be a form to fill passport details. That's all! The staff will be very helpful. Don't worry about that. They''ll give the forms and guide you.
what information we should take care about?
You'll take your original Passports (PP). And PP sized photos. Take your nomination letter and ITA letter too. Nothing else. Of course you credit card and/or cash to pay them! Some accept card, some don't. Call them up and confirm that. I can't think of any other information. Maybe if you have any health issues, feel free to discuss with the panel doctor there.
what forms they will ask ?
They'll not ask for any forms. If you are taking medical after CIC instruction, you'll have a form that came from CIC along with the instruction. So, in that case you'll take that form to the clinic.
If you are taking upfront medical, no forms are needed by you and you won't have such forms either. You'll take your PP and ITA letter and say you want to do upfront medical. They'll understand and do the rest.

EDE and Non-EDE are certain instructions for the panel physician. You don't have to worry about that. But when you get the tracking sheet after Medical make sure Non-EDE is mentioned there. Once you say you're doing upfront for economic worker class under provincial nomination program, they'll understand the rest and put you in Non EDE category. If the tracking sheet says EDE, ask them why and request them to explain a bit about EDE and Non-EDE and clarify whether or not they put you in the right category. You may ask this to the reception staff or clinical staff in the panel clinic.
Take a light breakfast. Have a good night sleep the previous day. Relax and take it just like you go for a cinema! You may spend 2 -4 hours there.
