Can you please share the no objection certificate( NOC) or affidavit format for applying minor passport in India from Vancouver BC(consular India) as the same is not present in their site? Pls help as this is urgent.

I got the affidavit format from the website. I have a doubt in filling up the form. What information do we need to fill in the last dash in the below form (passport with __________________). Please clarify.

I am in a situation similar to the one you have been in. It would be really helpful if you could tell what has to be filled in the last blank (passport with ________ )
