No guide available to fill out IMM Form 1411 Table A & B Family Tree - Canada

Hi there,

I have applied for my parents’ sponsorship to Canada and they received an acknowledgement receipt of application alongwith the attached form (IMM Form 1411 Table A & B Family Tree & Vertical Expansion respectively) last week in Pakistan. I have tried to search for a guide to fill out this form but was not able to find one. Please advise as what should be written in the box: “Family of” in Table A. If it is my father’s name then should another one be completed for my mother also? Should the deceased family member be included in the family tree? Table B is also not clear.

I am pretty sure its only for the principal applicant.

I got similar letter for my parents as well. In addition i got the request to fill out the following forms:
IMM008
Schedule A (IMM 5669)
Additional Family Information forms (IMM 5406)

Do you know which form is IMM008? Any link if you found it?

I have applied for my parents and received approval from Missisuaga and now received acknowledgement from Islamabad
they told us to send family tree form but forget to enclose it
can someone send me family tree form, even a link will be helpfull
because days are limited in which I have to fill it and send it back
any help will be appreciated

I have received a similar request for my parents and the sad part is that CHC-Islamabad forgot to attach the family tree form. I have tried searching the internet for a blank copy... even an idea to what it looks like. I have emailed CHC-Islamabad but i dont know when they will reply. If you (or anyone else reading this out there) has a blank copy of the form that i could have, i would REALLY appreciate it. my email address is noorieandi@yahoo.ca.
