This look likes a medical request to me, please contact panel physician and ask them to look your IME number in medical server.

Yes, If you did not receive an attachment with your email, do not waste time go to any CIC authorized panel physician doctor. The doctor will able to locate your IME number in system and do your medical. Best of luck
