I advise you take a look. You'll need your UCI, DofB and a few other details and you'll be just fine.

Renew it immediately...I'm sure you can renew before you submit your medical documents details.
Notify Accra once its done...possibly with CSE.You can also include a cover letter in your medical documents to Accra with the photocopy of your biodata page.
