If you had fill them as other names with the inclusion of a cover letter to explain it,no issues would have come up.
Regarding the explanation letter that accompanied your returned application,are you absolutely sure that your birth certificate is the acceptable type?Please check for the authenticity of it to be sure.
If you are sure,then we can now conclude that it's the names.
Two options that you can adopt are ....
1.Getting a new birth certificate from your local registration council in your area if it is possible.If it possible,then
2.A simple affidavit in the form i earlier described to you in my earlier post is suitable to remedy this issue.

Kollins, i'm very appreciative of your help. Just to answer your question. I verified the that the BC is of acceptable time, issued by the government with the government seal, names of my parents, my correct date of birth and my first name and last name as in other documents . The only different stuff is the middle name. Remember the return letter said i did not include the certificate and I did. It's right there , colored copy and clear. I'm going to a lawyer tomorrow to get an affidavit. I'll also inlcude the middle names I dropped in the alias section of IM0008. I also noticed that a UCI # was written on my IM008 form in pencil. This the first ever applying to canada , may be I can call with that UCI#. Thanks again

I would also like to add that my sister landed in June 2014. She also had a similar experience, but she still did her X-ray. They only perform chest x-ray while the abdominal region is carefully protected....btw she is a medical doctor so I believe she's informed.....However, it's still your choice....just contributing my 2 cents

Are the medical forms going to be filled by panel physician and handed back to me to be couriered to accra along with other requested documents or it will be sent to accra by the panel physician/ clinic? Any other person that has gone through medicals can pls answer this cos I plan to do my medicals at asaju medical centre with my family members soon. Your urgent response will be highly appreciated. Thanks.

Congrats on both the MR and our baby. I think u can do the medical tests so u can go and have ur baby there. Chest X-rays can be done during pregnancy without harming the baby and a lot of our seniors have successfully done it. It's ur decision at the end of the day. I would advice doing some research and seeking medical counsel cos we don't know what AVO will do later on.

Please house I have 2 more questions:

1. Considering Asaju is an e-medical center there is no need to take passport photographs along.

2. My last daughters passport was issued 2 weeks after which she was born hence the face on it is a baby face different from her face now. I hope the doctors would not say they have some difficulty identifying her face? There is a section on the form where they are required to say yes or no. Should i get a new passport for her before doing the medicals?
