In response to the CSE I sent Accra as regards a mail I received from them containing only my spouse medical instructions, Accra replied me today that in order to continue the process of my application, I need to supply the following :

1. An explanation of a depositor who made a deposit into my account. That is the bank statement I used in applying.

2. separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature Amended Schedule A. Please make sure you do not leave any gap in time. Indicate your past history since the age of 18. Specify what kind of work(s) you are doing and have been doing since then.

3. Bank statements for the past 6 months are required







4. Submit proofs that you have sufficient money to establish yourself in Canada.

5. Large deposits in your bank statements must be supported by evidence and explanation of the provenance of the fund.

I was told to get everything to them within the next 30 days to enable them continue process on my application.

Please, who can shed more light on the second item requested? Especially the part " bearing an original signature Amended Schedule A.".

Thank you everyone for congratulating me. The truth is that I did not expect this mr request at this time, at first I thought I was dreaming and my childred were asking me why was I talking to myself. All our expectations shall not be cut off. Must I wait for the forms before going for the medicals.

Don't really know what to say on this matter. I can only empathise. However, Sanmi13 are you the principal applicant or is it your wife? I am trying to understand the rational behind sending your wife MR without sending you. Please just take the time to explain things truthfully and meticulously to them. Always be consistent. As a member of this forum, it is the wish of everyone that we all scale through. All the best.

Glory be to God in the highest. I happily announce the MR blessing of God into my Life. I received my MR mail yesterday Oct 2nd in the evening. I really pray that God will answer every one looking up to him for such blessing speedily. I also rejoice with all those with their PPR may your joy know no bound in Jesus Name. For all those who are troubled as I was seeing the MR for applicants after me the lord will make it like a dream when the time comes remember "When the Lord turned the captivity of Zion, they were like those who dream"

I was asked to foward the following doc:
Medical examination for the entire family.
- Updated proof of funds.
- Updated police clearance certificates for you and your spouse.
