Hi! My mum’s final destination is Vancouver. But she will be transiting either through Montreal or Toronto. Is she required to give her molecular test and book her 3 night stay at a hotel at the transit
cities in Canada (montreal or Toronto) on her way to Vancouver or does she have to take a test and have a hotel stay at her final destination in Canada (van)?

Montreal or Toronto will be where she quarantines in the hotel and test and then once complete she can travel to Vancouver and finish her quarantine there.

Her application was approved for “exemption from the COVID-19 travel restrictions and limited release from the mandatory 14-day quarantine requirements for compassionate reasons”, does she still needs to stay at the designated hotel or she can quarantine for 14 days at the place where her support is required?

You are pregnant. Public health will not let people quarantine with pregnant women.
