New passport application

Hello everyone,

I'm in the middle of filling up my new passport application. Had a few questions.

-What should l select in the anticipated date of travel in the first page ? It has 2 options of month/day and Unknown. It is located below the box with the QR code.

-is it ok if I handwrite the date, city, province below the applicant signature or does it need to be typed ?

-l am using my health card as a supporting ID proof. My driving license has expired. So what should l enter for eye color and height. Its not mentioned in my health card or any other ID proof document.

3: Put your actual eye color and height. It is self explanatory.
