New Medical Procedur

U'll need 1 form:
1. Form IMM 1017 (For Principle Applicant)
(Note Forms for Dependent provided by DMP)

Here's the guideline/s:

A. When you and your family members go for your medical examination, make sure you have these forms completed for each person, along with the following items:

1. Passports for yourself and your family members, plus a photocopy of the bio-data page of each passport. If a passport is not available, provide an official identity document bearing the photo and date of birth of the family member;

2. Eye glasses or contact lenses, if you or your family members wear them;

3. Previous medical report(s), if any. You must tell the examining doctor about all past and present medical problems and conditions, and provide the doctor with any available documentation such as physician's reports, treatment or prescriptions; and

4. Five recent pictures [taken within six months preceding the date of the examination] for yourself and your family members (see section on photos in the instructions on how to complete the Application for permanent residence).

B. Make an appointment with one of the doctors identified on the enclosed list. A Medical Report, Section A (form IMM 1017) will be completed at the doctor's office for yourself and each of your family members undergoing the medical exam. A photo of the person to be examined by the doctor will be affixed on both copies of that person's form. This form is used to match your medical file to your immigrant file.

C. The doctor will sign the form and mail it to the Canadian Regional Medical Office (RMO) with the results of the examination.

I have visited DMP jeddah yesterday she told me they must need New IMM 1017 form with medical instruction witout this we can not do medical examination. no i am waiting from VO to send me that forms

If you are outside pakistan you need medical forms ,within pakistan you require "medical instruction"letter dispached /attached with e-mail .That contains the name and ID of the individual those undergo medical exam.For your convenience I am copy pasting some contents of the letter by CHC to one applicant.

Sorry to hear ......DMP require IMM-1017 it means that .......the form is essential for outside Pakistan....dear try to contact VO soon .......

So it means we will receive forms may be after few days

I have contacted the Designated Medical Practitioner in my city and in order to do the Medical Examination, they have asked me to provide them with the IME Number. They said without IME Number, I cannot do the Medical Examination! Visa office has already sent me the Section A of IMM1017 Form (05-1998) E, but there is no IME Number whatsoever anywhere in it, nor in their email. Can anyone guide how to go about this?

if u received Section A forms then u dont need anything more then that to complete your medicals and there is no IME number in section A there is only you file number and if your DMP still asking for this then batter to change your DMP because he dont know his job.

It is Paper base medical not E-Madicals good luck

Thanks Drooza! But what is that "E" at the end of the Form Name (IMM1017 Form (05-1998) E). Does it mean it is an E-Medical?
The visa office (Warsaw) has emailed the medical form (Section A only - one page) to my lawyers. So i am not sure if it s an E-Medical or any other type.
