New Medical Forms

Dear All,

My friend received medical instructions from London Visa office by email when he connect DMP they told him please bring Medical Report Forms which should come from Visa office with this email.

Please does anyone has this experience

He received one attached latter showing their Names and Clint Numbers. nothing more he sent email to London visa office but no reply.

Does he need more forms from Visa office or DMP does not know new procure.

When my token numbers was called I went to designated counter and show her all documents, after scrutiny the documents she surprised me that there is only one medical will be done because you have only IMM 1017 form CIC not send medical instruction for your family.....I was upset and surprise that how its possible......but I argue her that this is not possible may be medical procedure is change you should consult with you senior on this matter...at last she agree and went to her senior for further guideline I waited for her 30 minute and she came back with little smile and replied I was wrong...the procedure is change.....CIC only send Principle applicant IMM 1017 form with IME xxxxxsx Number and for dependants IOM will fill the form manually with Principle Applicant file no. before this CIC send all the forms to applicant and now all the forms provide by IOM for dependents. After this clarification she start working on forms one by one all the forms she filled, pasted photograph on forms attaching Passports copies and so on with original passports....she made separate folders for me and my dependants she also took some signature of me and my wife on forms . After that she asked me to pay the fees detail is as follows.

Under new regulations for pakistani applicants who recieve medical request online ,no medical form is required .just take copy of e-mail medical that contains ID number forward it to your DMP .rest would be done by DMP

No my dear friend...in new medical Procedure IMM 1017 is must...in whcih unique IME Number is issued with Barcode....for speedy medical procedure....if any one not get this form......pl read Medical instruction letter carefully in which they write that if any one not get attachment please reply the mail on the provided link......that mention in email.....if any body not get attachment please check medical instruction email carefully.........thanks

How to do Medicals w/out the forms?

Usually we need to do our meds thru the forms [IMM1017] supplied by our VO [the concerned CHC]. However, there could be reasons for doing the same WITHOUT them. Eg. the forms are lost, the CONsultant is withholding them, forms r somewhere & u r somewhere else etc. In such case, we 'can do the meds w/out the forms' too. Here's how:

1. Email CHC again to re-issue u the IMM1017 [med forms]... if time permits.
2. If not, Contact a DMP for an appointment.
3. Inform the DMP, in advance, that u want to do the meds "w/out" the forms.
4. They can get your meds done by 'creating' those forms* for u.

*Note:
- All DMPs have those 'blank'* forms.
- All they need is an 'authority' to do so. If u have the CHC's email - it would suffice.
- If u don't have an email from CHC, yet the DMP'll be happy to assist u [read they want the money].
- They'll need your File No. + 2xPhotos to prepare the forms [specific for u].
- U need to show the DMP your original Pps & pay the DMP's Fees [of course].

Important: If u do not get the CHC's nod to do the meds, remember the meds expire by 1yr & your visa/s will be valid till that period 'only' [except if anyone of your dependent's Pps expire before that, then it'd be earlier]. So, doing the meds early could be risky - as it might offer u lesser time to 'land'. If that's Ok with u, plz be free to get the meds done earlier.
