Hello po.. Ask ko lang po required po ba tlga na mgpvaccine? Yung hubby konpo kasi ngUpfront medical sya den sabi ng doctor balik po sya 2nd wk of july.. Ano po yung isasubmit sa cic na galing sa doctor? Do i have to send the original or ok lng to send photocopynpo? Salamat

-hello sis, c hubby q hnd naman xa nagpavaccine po nun.. original ung napasa po namen, pero photocopy nyo nlng ung e-medical note para meron kaung back-up for sure po..
