i have mailed my full appn to CHC,ND in first week the Feb, please find my time line in signature!

I have received Med, PPT request couple of weeks back, in the cover letter among the other things they said a ‘Foreign Travel Form' is enclosed, though it was not there in the envelope (I received the envelope torn form one side). I have written about missing ‘Foreign Travel Form' to CHC and am waiting for their reply.
I am writing this to know if CHC, ND has asked for 'Foreign Travel Form' to any member of this forum. Can you please e-mail me a scanned copy at hirentank@gmail.com?

Yes it is normal to ask the travel forms.

You can make a copy of it in MS Excel. Columns headings like SNo., Date of exit, County, Date of entry, Country, stay period. That is it.

my problem is - the CHC, ND has asked me to fill-in the 'Foreign Travel Form' (the one they have attached with the package), but that form is missing!
