You can download the form here http://www.cic.gc.ca/english/pdf/kits/forms/IMM5744E.pdf and you can request notes here https://atip-aiprp.apps.gc.ca/atip/welcome.do it takes 30 days and in some case it might come early but you will get a email with tracking number which will say 30 days processing time.

Also for your landing all you need is you pport and landing paper you received along with it

I'm also a April 23 applicant no remeds or any update
Just asked for additional documents on Mar 11, 2016, we submitted the documents on Mar 17, 2016 but still no reply or anything......

Just call your near by hospital which does medical for Canada they will provide u all info. You can not send medical even u want to, only & only your hospital can do that so take appointment now bro gudluck

I am one of those unlucky ones from July who hasn't received any updates after SA from NDVO yet. I have asked for an MP enquiry to be conducted? Should I also order notes? I haven't ordered any notes yet.
