They have mentioned the following:

Chest Xray (in DICOM / JPEG format) with report (in PDF)
Completed MEDICAL REPORT CHEST X-RAY REQUISITION AND REPORT form (Page 5 and 6 of IMM5419) (in PDF)
Duly completed Re-assessment Form (Attached in mail).

PCC of Italy and and fill out the form for details of foreign travel.

Yes, so I just sent them yesterday the Translations of PCC from Italy into English and all Travel History.Let's see how that goes.
For Re-medicals does my wife have to take any documents with her other than the Re-assessment Form or just show up?

Hi everyone. I need your suggestions.I did my remedical on 5 march.
After remedical I came to know that passport much be valid for atleast 6 months for visa Stamp. So I applied for the renewal.
Should I inform ndvo about the new passport.?? And if yes, when should I inform,before or after getting dm/ppr.??
Plz reply.

You should inform NDVO as soon as you get new passport, you have to again fill the new form IMM 0008 and send it to NDVO.

I really doubt that's gonna happen but the good news is that your remedical request is probably on its way!

U can order ur notes to get confirmation about ur medicals been update to ur file or not.....in ecas also u can see "medicals received" statement under permanent residency's "in process"....in few cases it is not updated on ecas.....best way to knw is thru ordering ur gcms notes to get information abt ur file....

ok great....even i did the same and my ecas also has 2 lines only.....just wait till they open the file. In case they don't receive..i was planning to send a mail to the medical id of ndvo to find my reports.
