We didn't get any attachment with the mail..

you said it was mentioned in email " duly completed re-assessment form ( PDF/Word)" right???
i received the same with an attachment of re-assessment form, which i submitted to doctor...then she filled in her observation. may be in hospital they might provide you. And then take copy of the email too

i have informed cic through mail and case specific enquiry, but did not get any reply from them

hi, doctors generally dnt provide dat form, many applicants never got it in mails, i hv observed it in this forum. it is a simple one page form.

OMG..!! Take care..!! Nd inform cic as soon as possible othervise u cant travel..!! It will create mess..!! Do it as soon as possible..!!

Chest Xray (in DICOM / JPEG format) with report (in PDF)
Completed MEDICAL REPORT CHEST X-RAY REQUISITION AND REPORT form (Page 5 and 6 of IMM5419) (in PDF)
Duly completed Re-assessment Form (in PDF/word)

What does it mean by, "Form, page 5 and 6 of IMM5419" Am I supposed to take those forms and give it to the physician, or do they it themselves?

hi tenso, nothing we need to do...its simple that u gotta have passport copies, original passport, passport size photos, email copy, reassessment form...once you show the email copy to them they will do the needed. even i dont know what form 5&6 ...may be that's hospital's work to fill and send.
