need help

i got a phone call from canadian embassy philippines. and told me that i can send my passport and photos and Appedix A form with it. but i have no clue where to get it and whats on it. they didnt specify what kind of form is it. its kinda like a passport request thru phone call.. please help me. i got deadline next month

It appears that you have not sent the document checklist of your local visa office with your full documents when you submitted them to Manila visa office.

Anyway, whether you have sent it or not, you need to comply with their requirments.

Print out the following document checklist which is called ( Appendix A) from page 2 to page 11, check the blanks in front of each item under column (F) if you are applying under the Federal skilled workers, and fill in the Personal Net Worth Statement page (9, 10, 11), and send them with your passport and photos.
