Need help with regard to new request from CIC

This is with regard to a new request from myCIC. I submitted my EE application yesterday and I have the medical passed and with new request.
1. 12345678 - My name (The number 12345678 is the UCI number from my FSW application and with my name)
2. Schedule 4
3. Schedule 4A.

Number 2 and 3 has to do with my province nomination and I will download them and upload the updated ones to myCIC profile.
But I am not sure of Number 1, what to upload for this request. Do i have to upload the letter received from CIC or do I have to upload more.

Thank you. I will send Schedule 4 and Schedule 4A.
But schedule 4A is not applicable to me. Do i have to upload the loe or upload the Schedule 4A with Not applicable written in sections of the form ?
