Need help to understand the Medical Surveillance Form

Hi there,

Can anyone help me about this Medical Surveillance Form? I didn't really know how to do this. Do you mind to enlighten my thoughts about this? What should I do? I'm here in NL and the embassy just emailed me to send them my passport to attach the visa but they will also send me this Medical Surveillance Form. May I know what this really mean?



Below is the email they sent.

In order to enable us to process your temporary work permit, we require:

- your original passport for visa issuance, please be advised that we will send the Medical surveillance form again once we have received your passport, please complete it and present it upon arrival at the port of entry.

PLEASE REFER TO YOUR FILE NO. IN ALL CORRESPONDENCE.

PLEASE PROVIDE THE REQUESTED INFORMATION/ DOCUMENTATION WITHIN 30 DAYS OF THE DATE OF THIS E-MAIL/LETTER. A FAILURE TO PROVIDE THE REQUESTED ITEMS MAY RESULT IN THE REFUSAL OF YOUR APPLICATION.


Does it mean that within that 30 days, I should send the form that they need? Or do I have to go back to NL after I'm done with this? I really need your help. Thank you so much!

Hi eva,

Do you have an Inactive TB reported in your previous medical?.

However, the matter you wanted to know is not so serious. You have to fill the said form and follow instructions.
After landing (not straight away) in Canada you have to contact on the given telephone number of health authorities. They will arrange for your chest X-Ray and some other tests.

thanks wilson..yes, they found out that i have an inactive TB..let say i have to enter canada by oct 20, does it mean that on that day i have to stay first in one of the ofce in immigration before i go to the place where should i'm staying in canada? thanks again.

Hi evz,

You can go and live where you have already planned. You will have instructions from Visa Office along with your COPR and other papers.

Upon your entry to Canada, Port of Entry staff will send your copies of IMM 0535 forms to the Medical Surveillance Unit (MSU) so that provincial and territorial health authorities are made aware of your arrival who requiring medical surveillance. You may be asked to report to Public health Authority within 7 or 30 days as directed.

Thank you so much for all your words of wisdom about this matter. It really helps a lot. At first, I'm really worried with this case but because of your explaination all my worries are gone. I'm expecting my visa next week and as long as I have that, I can book my ticket to Canada.
