need help to fill up Caips form (IMM5563E)

Hi Everyone,

I am trying to order Caips notes for myself through a friend of mine who is a Permanent Resident in Canada. Though, it has been discussed before in other threads (Qorax and Cocknbull had started 2 very useful write-ups about how to order caips notes), I am nevertheless having a little bit of trouble filling up Form IMM5563E http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf

Question 1:
a. Section B of IMM5563E has space only for details of P.A (Family name, Given Name, D.O.B). Where do I mention my wife's details?
b. I am assuming I need to tick electronic file and not physical files. What are the respective boxes that I need to tick and details that I need to fill? (My Location would be New Delhi Visa Office and Client ID would also be filled).

Question 2:
In all, I need to send out IMM5563E (Signed by the requester), 2 IMM5475E consent forms (1 with my details and another with my wife's details) and then the receipt of my online fee payment of $5 with the details.
Do I need to send out a separate cover letter or will the above forms suffice?

Question 3:

What all documents should the requester send to show proof of his residence?

1. only pa's info needed.
2. ya, elec. file: n info is jst needed to fill in box of the respective option.
3. copy of resident card will do.
