Hi!

I need help on filling-up the schedule A of my spouse.

1. UCI

When I received my Sponsorship Approval I got our file number and a UCI and then, when my husband got a request letter from CIC. On the letter, I saw a different UCI.

I am filling-up the forms and I realized, what UCI to use. The one in the SA or in the request letter? plus, when I tried to put the UCI from the request letter it doesn't accept it because it has dashes (-) in between numbers. ---> like this 12-1234-1234

2. Request letter

On the request letter they told us this instructions:

o Please ensure that the form has been completed and that the information you have
provided in Section 6 Personal History (2014/08 – 2015/06) is reflected in Section 10
Addresses for each time period (2014/08 – 2015/06).

What could this mean?

My husband only lived in one address since birth that's why he used only one row under:

Section 10: Addresses
2009-03 - Ongoing

then, in section 6: Personal History

2016-04 Ongoing
2015-06 --- 2016-04

2014-07 --- 2015-06 (The one CIC noted on the letter) this timeline here is updated before it was (2014/08 – 2015/06)

2014-02 --- 2014-07
2013-08 --- 2014-02
2012-03 --- 2013-08
2009-03 --- 2012-03

-----

NOW, On his Edited Schedule A, Every time period on his SECTION 6: Personal History is reflected on SECTION 10: Addresses but having only one address (SAME ADDRESS ON EACH ROW -- because he never changed address or moved anywhere else)

3. Vacation

My husband had an out of the country vacation in 2015 for more or less 5 days (stayed in a hotel). Do I need to put it on the SECTION 6: Personal History? and if I have to, What address do I need to use?

4. ADDRESS

On filling-up the address. What is the District? and we have Province but when I chose "Philippines" on the drop down menu, the state/ province is blocked.

On his ECAs there is "NOT APPLICABLE". I think there is something wrong and I need to correct it too...

This is on the ECAS:

115 A. Mabini street
Baliuag, Bulacan C-3006
NOT APPLICABLE Philippines

...and this is the correct address:

Street number:
#115
Street Name: A. Mabini street, Sabang
City/ Town: Baliuag
Province: Bulacan
Country: Philippines
Postal Code: C-3006

Thank you so much for all the help!

1. The UCI on SA belongs to the Sponsor. The UCI on the request letter belongs to the PA, your husband. Use your husband's UCI and don't put the dash (-).

2. As earlier answered on the other thread, make sure there is no gap on Personal History. When did he start working and how many jobs he's had from the time he started working? If he worked or is working in the same "district" or "city/town", then it should not be an issue anymore with CIC as it would match to the "address" where he has been living.

If he has lived in the same address since birth, then there should only be 1 row filled under Addresses.

3. No.

4. You might want to fill it this way:
Street number: #115
Street Name: A. Mabini, Brgy. Sabang
City/ Town: Bulacan
District: Baliuag
Country: Philippines
Postal Code: C-3006

Thank you!

Yes, He's living in one address since birth. I am just wondering if they put that kind of instructions which pertains exactly on one of the rows in Section 6?

2016-04 Ongoing --- UE
2015-06 --- 2016-04 --- Same work different position. (KIA)

2014-07 --- 2015-06 --- (KIA) (The one CIC noted on the letter) this timeline here is updated before it was (2014/08 – 2015/06)

2014-02 --- 2014-07 --- UE
2013-08 --- 2014-02 --- E
2012-03 --- 2013-08 --- UE
2009-03 --- 2012-03 --- Study

I am thinking they were got confused because he is in the same job on both rows. BUT I am pretty sure they read the position/ Job were changed...
Plus, we submitted his Schedule A form via Webform early April because we did not receive any correspondence from them. My husband only put ONE ROW on his SECTION 10: ADDRESSES. Then, we got that correspondence from them saying:

o Please ensure that the form has been completed and that the information you have
provided in Section 6 Personal History (2014/08 – 2015/06) is reflected in Section 10
Addresses for each time period
(2014/08 – 2015/06).

What do they mean by this instruction? :(

---

On the Vacation part. Is it okay? I mean, He has a stamp on his passport indication that he went out of the country... I am just worried that we might got question on why we did not declare his trip...

---

I noticed on the old IMM 5669, the State / Province / District were all in the same box. Which mean we can put our province on the text box. but then on the latest Sched A. It was all different. We got confused too on filling-up the address area our Generic Form. I think we have a problem on that category which is why there is "NOT APPLICABLE" on his address.

I can follow that format too. While, I asked another applicant on how they filled-up their address, They told me she did not out anything on the District textbox and now they are DM. They are Feb applicant too.

Thank you so much for all the help. Hoping to see your response soon.

2014-07 --- 2015-06 - you updated this after you received the letter from CIC about Sched A?

================

On the Vacation part. Is it okay? I mean, He has a stamp on his passport indication that he went out of the country... I am just worried that we might got question on why we did not declare his trip...

CIC is after those who travelled who had or accumulated at least 6 months or so in a particular country. Your husband had less than a week (as you mentioned earlier) which is obviously just a leisure trip.

I had several trips in the past which were all under a week and never declared them in my Personal History.

================

I noticed on the old IMM 5669, the State / Province / District were all in the same box.

Street number: #115

Will leave it up to you if you want to follow the above or make the City/Town to Baliuag, Bulacan and leave District blank. Or, you could put "2nd District" under District since Baliuag is under the 2nd District of Bulacan.

Hi!

That what happened to us. My husband filled out his Schedule A and then print it to hand signed the from then, he scanned it.
The scanned copy was the one you will send via Webform. It will take time before they response so, just be patient. I know it's a bit hard not to panic or worry because of the silence.

Now, after two months, they asked us for another copy of His Sched A. We then received a correspondence via My CiC account. We can finally upload it.

This is the WEBFORM where my husband first sent his Schedule A. Yes, they will send a correspondence (which we were supposed to receive 5 business days after March 22 but we never received it). So, my husband sent it via webform, stating that he did not received any correspondence on his CIC account.

I called so many times on the hotline because I am so worried because they were not responding and when I talked to the agent, they said that the Schedule A wasn't attached on the file still. They told us to wait because there is a bit of a backlog on the IRCC Webform so it will take time for us to get a response from them...

So we waited.

...Until May 30th we received his FIRST ever correspondence on his CIC account. It was a Request Letter - The letter only says that he needs to submit his Schedule A again. VIA CIC account where the upload button was then activated. I just finished his Schedule A and I just literally uploaded it on the CIC Account hoping that they will received it today (Monday).

VIA WEBFORM:

Fill-up the form
Print it and then sign it
Scan the form
and then send it through IRCC Webform

It seems there is really a backlog right now --- based on your timeline , you waited for two months before you received this correspondence and since you submitted the application earlier, ( we submitted the application April) , I presume that I might not receive a correspondence from them soon
( uploading of Schedule A)... thank you so much for entertaining my queries Anna... hoping and praying for a positive response for all of us:)
