Apart from Applicant Information form, finger print paper and cashier check,
are there any other forms need to be filled up? like "Authorization to release information" document from CIC site and cover letter etc....????

1. Nope, once you receive it, you send it to CIC or include it in your application.

my question is not for CIC, to FBI, to get the clearance certificate from FBI, what else need to be sent to FBI?

No Cover letter required for FBI. If you had filled the FBI finger print form with required details that suffice.

If you are doing the finger print your self you need to do it properly.

I got my FBi certificate, but my wife certificate was not provided as the prints were not clear. If you are not sure you can attach up to 5 finger prints for them to take the right one. (Yes i got a mail from FBI with that details, so up to 5 allowed)

Hi Bolla,

YOu need to attach Finger print form, Payment form and then a application form. Have a look on this below site:
