Need help in filling form IMM5406

Hi,

In the guide to fill the forms it is mentioned as

Additional family information (IMM 5406)
Who needs to fill out this application form?

This form must be completed by:

You, as the principal applicant,
Your spouse or common-law partner (whether accompanying you to Canada or not), and
Your dependent children aged 18 or over (whether accompanying you to Canada or not).


My husband is the primary applicant and i am the dependent. We don't have any kid more then 18 years of age. As i understand My husband and I we both have to complete separate forms of IMMM5406. Kindly confirm.

As you said my husband and i we both have to fill a separate form. So in first form (Under section A) my husband will write his name as applicant and my name as Spouse

Here my doubt is when i will be filling the form how i will mention my name and my husband's name in Section A. Please help me.

I have the same question. The instructions are not clear on this.

Please let me know if you sorted it out and how you did it.
