Education : Masters as per ECA report
Work experience: 4.2 years

Dec 2009 to June 2013: NOC 1211 (Part time)
June 2013 to August 2015: NOC 1211 (Full time)
April 2016 till date : NOC 1223 (Full time)

Funds: 12,500
Age: 29
IELTS: Listening : 8.5 Reading: 6.5 Writing: 6.0 Speaking: 6.0

Please suggest

I think the person must achieve the 67 pass mark for the FSW eligibility test first.

It could be because of the primary occupation that you are mentioning does not meet the FSW requirement of minimum 1560 hours of continuous experience in the same NOC.

Also, it could be because you do not score 67 out of 100 points on the FSW grid http://www.cic.gc.ca/english/immigrate/skilled/apply-factors.asp
 
Isn't IELTS should be 6.5 minimum in all bands to qualify ?

You must meet the minimum level of Canadian Language Benchmark (CLB) 7 in English or Niveaux de compétence linguistique canadiens (NCLC) 7 in French for your first official language in all four language abilities.

True, your primary noc would have to be 1211.
