Need help filling IMM5669

Hello,

IMM5669 is asking to fill out Education History and Personal History. my child is 5 months old. What should I write ? I AM SPONSORING MY CHILD.
Also in Question 3, MOTHER DETAILS, it is asking for family name at birth. So whose birth ? Mother's birth ? or the child's birth ?
This sponsorship application is for my child.

Also, Does IMM5604 APPLY TO ME ? Me and my wife both are PR. I am already in CANADA and my wife is also PR but will join later.

You fill in imm5669 from the point of view of the child. So the mother's maiden name is the relevant response here.

imm5604 does not apply as both parents are PRs. To avoid any misunderstanding it will not hurt to include a short letter saying you have not competed imm5604 as both parents will reside with the child in Canada and are already PRs.
