Need feedback of my application

Hi,

In 2016, I applied for open work permit visa and it got rejected. The consultancy person has submitted their email id and phone number in the application during processing, so I do not know the exact reason for rejection. They are not in contact now. They have not communicated anything about my reason for rejection. I want to know the feedback on my application. Can anyone please help me on this query ?
I only have the tracking id of my application but not the application number.

you need some information e.g. Receipt of fee payment , UCI or Application number at least to apply for case notes
