Need clarification on citizenship application questions

I am in the process of applying for citizenship for myself and my wife. I have some confusion regarding some questions, would appreciate if anyone can help.

Question # 6: List all the names you have used, including name at birth, maiden name, previously married names,, nicknames?

Query-1: In my case, I never used any other names so I guess I have to fill first row (last name and first name) and from dropbox select name at birth, is that Correct?

Query-2: In my wife's case, before marriage, her last name was her father's family name. But, after marriage her last name was changed to my first name and since then it has been used every where on all immigration documents and passport because we applied for immigration after marriage. However, I just realized while looking back in my immigration document for dependent applicant (my wife), there was a question in the form for dependent applicant, if you have used any other name, nick name, maiden name, alias, if Yes, mention below, if NO, then leave it. We selected NO and didn't write anything.
Now, when I am filing citizenship application, what should I do in this case, do I need to fill second row for her with the details of her previous name and select maiden name from the drop box? For the row-1, I will write her name as of current legal name. My question is name before marriage. Please let me know.

Question #11: Tell us what you have been doing your eligibility period inside and outside Canada?.

Query-3: Do I need to inform people that I am using their contact name, phone and email addresses in my application or not required?

Query 4: I used to work at SEARS which is no longer in business. What should I write in the field where name of the person, phone number and email address? Shall I put (Not available) or write or Not applicable as the company does not exist.

Query 5: I used to work at a bestbuy store, my senior and all colleagues have done, I don;t have the name and phone number and email address of the person, shall I say Not available or Not applicable?

You need to put atleast your last HR Manager or supervisor email address and phone number . or the address and phone number of the location you worked. and dept. They could sometimes ask you to send your paystub atleast to verify. They very well know SEARS is unoperational.

Put your Best buy work location address and phone number. Here too you will need some documentation on hand like offer letter,pay stub

Regarding querry-2. At the time of immigration documents submission, I had submitted our marriage certificate and her birth certificate. On both of the documents her name is (nazia khalid) which is her name after marriage and the same has been used every where on all identities after marriage and after immigration. Actually, it was a mistake on birth certificate and marriage certificate and I just found recently as her name on both (marriage and birth certificates should have been (nazia walid). So which is why I am concerned. I am thinking to put N/A for her. Any suggestions?
