NDVO PPR Next steps

Hi

I just got the PPR from the NDVO.

I went to the VFS website and its apparently asking me to fill out a form and attach it with my passport apart from the consent form. I have the consent form.

My question is apart from the consent form, which other form is to be attached? Can someone please provide me the link to the forms to be attached with the passport since i couldnt find it on the VFS website.

Step 2 :
Fill out the application form and other relevant documents electronically and print.

Forms (including the barcoded sheets) are to be printed on a white, bond quality, non-glossy paper, using a laser printer.

Also apart form the passport and the 2 photos what other documents are to be submitted

There will be an Appendix B, that comes with the mail. You have to print and fill that.
