It is not necessary that officer step up ahead. Can also send direct PPR.

Which process takes long time ? Security check or officer verification time at ndvo ?

Does they operate file in some pattern like surname wise , statewise or pnpwise or just random ?

First preference usually Express Entry, Refugee Application and then rest. Mostly which I noticed recently when was " file recieved ". Security checks takes longer time if stayed in multiple countries longer then 6 months. I don't think anyone can comment on officer verification time.
